Please complete the form below to request assistance from the FVSD IT Department.

To help us resolve your issue as quickly as possible, please include as much information as you can, such as:

  • Student Name

  • Student ID Number

  • School Name

  • Detailed description of the issue

If your request is related to a Chromebook or iPad, please also include the Asset Tag number.
You can find the Asset Tag:

  • On the barcode sticker on the back of the device, or

  • In the top-right corner of the Chromebook login screen

Please include any screenshots, photos, or attachments that may help us better understand the issue
